The UMID Card or Unified Multi-Purpose ID Card encompasses the SSS, GSIS, PAG-IBIG, and PHILHEALTH programs. Instead of showing the different valid ID cards for transactions, the Unified Multi-Purpose ID (UMID) Card can shoulder those IDs in one card. The purpose is to reduce Filipino people’s expenses and avoid redundancy in their database.

The Unified Multi-Purpose ID (UMID) is one of the most potent identifications in the Philippines since the UMID Card security features have a first line of defense against those who attempt to defraud their system.



The initial security features of Unified Multi-Purpose ID (UMID) evolved into a contact-less smart-chip technology, which stores the cardholder’s complete identity biometrics data. Including the facial image, digital signature, and fingerprint print-out in a computerized on a yellow piece of cardboard.

REQUIRED DOCUMENTS FOR UMID CARD MEMBER

1. SSS Members

  • Members have paid at least one (1) month contribution.
  •  Valid the date of coverage (Indicates when you started as an active SSS member).
  • No existing SSS ID /UMID.
    • For Kasambahay or House helpers without posted contribution/s:
        • Must be permanent the membership status of House Helper.
        • The employer must present the proof of contribution in the branch teller’s section. [SSS Form R5: Download Here].

2. GSIS Members

  • All pensioners and GSIS active members are required to get an eCard/UMID.
  • Bring two (2) government-issued IDs: (e.g., Postal ID, Voters Registration, LTO License, Passport, PRC ID, etc.

3. PhilHealth Members

  • UMID card is also available for those PhilHealth members and indigent [see here the list of indigent] people who have insufficient income for family subsistence which is not covered by either SSS or GSIS members.




STEP-BY-STEP PROCEDURE TO GET AN SSS UMID CARD

A. UMID CARD THROUGH SSS MEMBER’S REGISTRATION

1. SSS UMID Application Form

  • Download and print the application form. [UMID Card Form: Download Here]
  • The SSS Number, Name (first name, middle name, last name), date of birth, and gender/sex must be the same on their database.
  • Fill out your complete details, including:
    • Use a black pen.
    • Provide your permanent address. (The address you provide will be the reference for delivery of the SSS UMID. So make sure to write the correct address)
    • Provide at least one (1) contact number (Mobile, Landline, or e-mail address) to notify you of the status of your application.

2. Valid Government-Issued IDs

  • Provide at least one (1) primary government-issued valid ID.
    • LTO License
    • PRC Card
    •  Passport
    • Seaman’s Book (Seafarer’s Identification & Record Book)Alien Certificate of Registration
        • There’s an option if you cannot provide those primary IDs above. You can submit two (2) secondary ID/documents bearing your correct name and at least one (1) with a valid date of birth.

3. Submission of Documents

Submit all the documents and check below the available SSS branches nearest to you.

4. Photo Capture and Biometrics

After you submit all the documents, the assigned staff will verify your account. After that, the following procedure is biometric, which contains your photo, fingerprint, and a digital signature stored in their SSS data center.

5. SSS UMID Card Delivery

One of the most prolonged processes of valid ID to deliver in the Philippines is the SSS UMID. Usually, It takes 7 to 10 working days to deliver outside Metro Manila and 10 to 20 working days in any remote province. However, in Metro Manila, they will provide the UMID for 5 to 7 days.

For some areas, usually, the delivery of the UMID card takes 30-60 days, so do not expect it to deliver on time.

6. SSS UMID Card Activation

The final step is to activate your SSS UMID card in any nearest SSS branch.



B. UMID CARD THROUGH GSIS MEMBER’S REGISTRATION

1. GSIS UMID e-Card Enrollment Form

2. Valid Government-Issued IDs

Bring your two (2) Valid IDs and the accomplished GSIS UMID e-Card Enrollment form.

3. Photo Capture and Biometrics

After the review of your application, you will now undergo photo capturing, fingerprints, and digital signature.

4. Application Process

Once you submit all the requirements, it will process your application. Assigned staff will advise that you will wait for a text message where you can claim your UMID card.

5. Release and Claim the UMID eCard

Go to the GSIS office where you made your account and bring the following.

  • Release form of UMID eCard
  • Two (2) Valid IDs, Passport, and GSIS eCard. (Must bring original and photocopy)

6. UMID Card Activation

Like bank ATM cards, the UMID card also needs to activate, and the procedure is relatively easy. Here are the easy three steps you must follow:

Step 1: By activating your new UMID card, you need to visit the nearest GSIS branch or look for the [email protected] kiosk in city hall, municipal hall, or Robinsons Malls.

Step 2: Register your pre-registered fingers to the touch screen so that the [email protected] device will recognize and scan your pre-registered fingers, and it will be able to store your data information.

Step 3: A Confirmation message will notify you on the screen, informing you that you will activate your new UMID card in the next 72 hours.



THINGS THAT YOU NEED TO REMEMBER

  1. Social Security System (SSS) released a warning to their clients that do not entertain any persons or someone telling you that they are one of the staff in SSS offering to you for a “rush” application and asking for a fees change to a fast process of UMID. SSS inform the public that the only way to obtain the UMID is “first-in-first-out” basis, and if you encounter “fixers,” don’t hesitate to report them to the SSS hotline at: 8-1455 (for landline) 02-1455 (for mobile), SSS Call Center Numbers (632) 7917-777, SSS Toll-Free Number (632) 8920-6401.
  2. Check that all your details like your complete name, address, and phone/mobile number are correct, updated, and working. So the mail carriers will quickly locate your location to deliver your UMID.
  3. Make sure that someone from your family receives the delivery since the mail carriers require that someone sign their receiving copy. If no one receives the parcel, It will be sent back to the post office.
